    We have the ABO/Rh confirmation order built so that it can only be ordered by lab staff when needed. We don't allow surgery staff to put the order in. Blood Bank knows when we get an order if a second type is needed and this prevents OR staff from drawing two tubes at the same time and trying to say they were drawn at different draws.

    @Neil Blumberg, I wish we had you at all of our facilities to educate our medical staff. Sadly, convincing Hematologists and Oncologists (at least here in America) that it is better to postpone platelet transfusions than give ABO incompatible platelets is, more often than not, rejected, especially in light of the fact that many patients are having to wait because of lack of platelet inventory to begin with. 
    What we really need is a push for better transfusion therapy education in medical school. Along with this, continuing education for practitioners needs to become a priority. It is, however, quite difficult to get time with these practitioners. Even when we convince our laboratory medical directors to advocate for these issues, in my experience, clinicians rarely change. 
    All that said to say, in the "trenches," the practice will likely continue to prioritize inventory over safety.

    We receive cord samples on every baby and only test the ones (ABO/Rh DAT) from Rh negative moms or those with a clinically significant antibody. Our LIS allows us to see both mom and baby demographics  at sample log-in. We never perform an eluate from the cord.

    All babies born to Rh Neg moms get an ABORh and DAT IgG.  We still have some pediatricians who order on O pos moms.  If the DAT IgG is pos, we do an eluate only if the physician requests.  And they don't request.

    We only routinely get cords on Rh negative moms & moms w/ a clinically significant antibodies.  Occasionally a physician will request one on an O mom who has had a previously affected infant, but it’s not very common.  If a baby didn’t have a cord performed, and there are concerns of jaundice later, we have a separate test order for the ABO/Rh/DAT  drawn via venipuncture or heel stick.
