JShepard, I have been approached on this subject multiple times.
There might be a loop hole. If the Nursing staff call it a "blood draw", it doesn't fall under the QSA regs for BB.
If the nursing staff call it a "therapeutic phlebotomy", it does.
What's the difference between a blood draw and a therapeutic phlebotomy?
Nothing, except what regulations fall under it.
The result is the same.
I have been telling my nursing staff to call it "blood draw".

I am enormously honoured to announce that I am going to be awarded the Gold Medal of the British Blood Transfusion Society at their Annual Scientific Meeting in Brighton this year. It is awarded to an individual for their exceptional and long standing services to the Society and to the practice of blood transfusion in the UK. Sorry if this sounds egocentric, but I am very excited.

We now use MediaLab. We loaded all of our current procedures into it. To edit you download and make the change. Once the new version is uploaded and approved by manager/Med Director major changes are sent to all personnel that use the procedure for review. Each step is documented electronically along with a note that you can add to describe the update. The person making the change is notified to remove old paper copies from whatever manuals you have it living in. You can make links available to the Intranet for nursing, etc. Every 2 years if no changes are made each procedure is sent to manager/Med director for review. It is a great system and user friendly. The old procedures are archived.

We use MediaLab as well. Great for document control. They also have something called Inspection Proof, which makes CAP inspection much easier for you and your inspectors. This is cloud based, not expensive.

I believe.....Thermometers are placed in solutions similar to the components of blood.
10% glycerol solutions is determined to be similar to blood.

#6 is incorporated into the approved DHQ and is required.
We had the other 5 that you do. We did get rid of the "any Medications" as we ended up documenting tons of non-value information. Took up tech time and memory in our app. People take too many vitamins, supplements etc these days that have no impact on donation.

Only moderate or severe AEs require MD review. All minor events are reviewed by nursing staff to ensure that they're properly categorized and were appropriately managed. (Moderate/ Severe are categorized as LOC, additional medical intervention (call 911)...)

We use Velcro shelf labels. For the part that gets stuck to the fridge / freezer, we use Gorilla glue.

That's a great idea, but our shelves are stainless steel and magnets won't work.

That's the way we've always done it but I'm planning on changing it (one day..) after reviewing the verbiage of the applicable CAP checklist requirements.

TRM.42750 Storage Unit Alarms Phase II
All component storage units are equipped with an alarm system that is monitored 24
hours/day (in laboratory or remote), with documented alarm checks (for both low and high
settings) performed at least quarterly.
NOTE: The laboratory should be able to demonstrate how the alarm system works and that there
is a process to ensure a timely response to an alarm, including remote alarms.
Evidence of Compliance:
✓ Records of alarm checks documented at defined frequency
We use an automated electronic temperature monitoring solution. Naturally all of the storage units have built-in alarms as well. All of our blood/tissue storage units are in the same area as our technologists. We do a daily alarm check on the built-in alarms.
Probably a good idea to do an automated alarm activation check after calibration/upgrade/repair/new probe/etc but I don't think we need to waste our time with quarterly checks on the automated system, especially with how annoying it can be to do.

I'm not aware of any requirement for the Medical Director themselves to review the actual collection records.
I'm wondering if this is in reference to CFR 640.3 (a), where it says: The suitability of a donor as a source of Whole Blood shall be determined by a qualified physician .... or by persons under his supervision and trained in deterining suitability......etc Read on in the CFR and there is an explanation of how/when other staff can perform certain duties.
Likewise AABB standards indicate the Medical Director is responsible for activities....but they may be delegated to other personnel. Std 1.1.1

We use the TempSys wireless temperature monitoring for our equipment, but we have a thermometer in there for a backup.
It's a Temp-Chex (made by Streck), Enviro-Safe, thermometer for incubator. Cat# 240053. It's spirit filled and has a small bottle filled with glycol solution.
