Transfusing past expiration time

Hey out there! I'm just wondering what the common practice out there is on this: Do you dispense units within 30 minutes of the product expiration time? The computer allows us to dispense the product, but what is the SQuIPP of the products if transfusion continues past the expiration time? Do we need to be concerned of bacterial infections?

Just a caution - you might also warn your techs not to tell the RN that "the unit must be infused before the expiration time too". We occasionally issue a pltph just before midnight and I had to make sure my techs were not doing this. Some of them had "made up that rule", but with RBCs it is especially dangerous to shove the unit in too fast just to beat the expiration time. We tell the RNs that the unit must be picked up before midnight (or the computer will not let us issue it anyway) but that they have the normal 4 hours to give the unit. Seems to work.

Per the most recent edition of the "Circular of Information" (recognized by FDA) Edition dated December 2009,

p.5

13. "Transfusion should be started before component expiration and completed within 4 hours."

This was a revision from the previous edition, which required the product to be infused prior to the expiration.
